  • 2021 941 Employer's Quarterly Tax Returns for 1st, 2nd, and 3rd quarters only

  • 2020 Detailed Payroll Journal for all employees for all quarters

  • 2021 Detailed Payroll Journal for all employees through 9/30/2021

  • Provide date and amount of PPP loan/grant disbursements (1st and 2nd draw if applicable)

  • Health Insurance Composite of all Employees who receive medical benefits and the cost to the company for 2020 and 2021

  • For NC Businesses Only: Submit NCUI101 instead of payroll journals.
